i. individual applicants:
a. applicant’s name and address;
b. a copy of the relevant pages of the applicant’s passport along with documentation to
confirm his/her Bahamas immigration status, if any.
ii. corporate applicants:
a. applicant’s name (including trading name, if applicable);
b. a copy of the applicant’s formation document (e.g., Certificate of Incorporation,
Partnership Agreement, Declaration of Trust, etc.);
c. a copy of the relevant pages of the passport along with documentation to confirm the
Bahamas immigration status, if any, for each of the applicant’s beneficial owners.
iii. confirmation of the amount and purpose for which the Bahamas Dollar notes are required;
iv. any other information considered pertinent to the Central Bank of The Bahamas’ consideration
of the application.
